Who is a candidate for All-on-4 or All-on-6?
These techniques are designed for patients with full edentulism or imminent tooth loss (loose, infected or extensively fractured teeth) who want a fixed, stable, long-term alternative to removable dentures.
Missing, mobile, infected or condemned teeth on one or both arches.
Instability, pain, nausea or discomfort with a conventional removable denture.
All-on-4 can often avoid bone grafts thanks to tilted posterior implants placed in denser bone.
All-on-4 vs All-on-6: what are the differences?
The choice between the two techniques depends on bone volume, occlusion, parafunction (bruxism) and prosthetic objectives — determined after CBCT scanning and 3D digital planning.
- Avoids grafts and sinus lifts in many cases
- Shorter overall treatment duration
- Ideal for limited posterior bone density
- Immediate loading often achievable
- Most economical full-arch solution
- Wider load distribution and greater stability
- Recommended for bruxism patients
- Suited to longer prosthetic spans
- Maximises use of a denser bone structure
- Enhanced long-term biomechanical security
| Criterion | All-on-4 | All-on-6 |
|---|---|---|
| Number of implants | 4 (2 tilted) | 6 (axial) |
| Bone grafting required | Often avoided | Depends on anatomy |
| Bone density needed | Low to moderate | Moderate to high |
| Severe bruxism | Management required | Better tolerance |
| Immediate loading | Often possible | Possible per stability |
| Total treatment duration | Slightly shorter | Similar |
* The definitive plan is decided by the implantologist after 3D CBCT assessment and occlusal analysis.
5-step treatment process
Photographs, digital impression or intra-oral scan, CBCT 3D scan. Analysis of bone volume, occlusion and smile line. The implantologist determines whether All-on-4 or All-on-6 is indicated.
First stay · Day 1Virtual implant positioning using 3D software, prosthetic wax-up to validate aesthetics and phonetics before surgery. Fabrication of a custom surgical guide where indicated.
Custom surgical guideExtractions if required, implant placement under local anaesthesia or conscious sedation. If primary stability ≥ 35 Ncm, a screw-retained temporary prosthesis is fitted within 24 to 72 hours (immediate loading protocol).
First stay · Days 2–7The temporary prosthesis is worn for 3 to 6 months. Regular remote check-ups by video call and WhatsApp: photos, hygiene guidance, prosthetic adjustments if needed.
3 to 6 monthsAfter full osseointegration, a definitive screw-retained prosthesis is fabricated: monolithic zirconia, layered zirconia or titanium-resin depending on the clinical case and aesthetic preferences.

The cost varies depending on the number of implants, prosthetic material, any bone grafting required and immediate loading. A personalised quote is provided after 3D planning. The ranges below are indicative for one full arch.
What these techniques offer — and their limits
- ✓ Fixed, stable prosthesis: vastly improved chewing, speech and comfort vs removable dentures
- ✓ Reduced need for bone grafting (All-on-4 with tilted implants)
- ✓ Immediate temporaries possible in many cases
- ✓ Natural aesthetics and harmonious smile
- ✓ Long-term function depends on oral hygiene, maintenance, health factors and clinical conditions
- ✓ Full care coordination for international patients
- ! Rigorous daily hygiene is essential (interdental brushes, oral irrigator)
- ! Smoking, uncontrolled bruxism, unmanaged systemic conditions must be assessed
- ! Bone grafting or sinus lift may still be required depending on anatomy
- ! Immediate loading is not guaranteed: subject to primary stability at surgery
- ! Two stays required: surgery + definitive prosthesis 3–6 months later
Maintenance & long-term care
The longevity of your All-on-4/6 arch depends directly on the daily care given to the prosthesis and the implant sites.
Twice-daily brushing with a soft brush, interdental brushes adapted to the implant-prosthesis interface, and daily use of an oral irrigator (water flosser) to clean underneath the prosthesis.
Visits every 6 to 12 months: prosthesis removal, screw torque check, professional cleaning and implant review. Radiographic assessment included.
A night guard is strongly recommended for bruxism patients to protect the implants and prosthesis from excessive nocturnal forces.
Smoking impairs osseointegration. Uncontrolled diabetes and long-term corticosteroid use should be monitored with your referring physician.

All-on-4 & All-on-6 patients at EstheticaPlus
After years of struggling with removable dentures I could no longer tolerate, I chose All-on-6. The team planned everything in 3D and I had my temporary teeth within 48 hours. The final result is beautiful. No regrets.
I was worried about the surgery but everything was completely calm. The 3D planning reassured me from the start. My bone volume wasn't sufficient for All-on-6, so they chose All-on-4 without a graft. Perfect result.
Very professional clinic with state-of-the-art equipment. Coordination for both stays was impeccable. The team answered all my questions via WhatsApp throughout the entire healing period.
